Education Fund Forms

• Form A: Local League Education Fund Project Approval Request
  Use this form if you want to use your own Ed Fund money for a project in your Local League. LWVWA Ed Fund must approve these projects prior to the start of the project due to IRS requirements. 
• Form B: TRY Withdrawal Request
  No prior approval is needed for the use of funds for producing, printing, and distributing They Represent You (TRY) publications.
• Form C: PMP Withdrawal Request
  No prior approval is needed for the use of your local League funds for paying PMP (Per-Member-Payments) to LWVUS. Local Leagues cannot use Ed Funds to pay state PMP.
• Form D: Local League Education Fund Final Report
  Once your local League has completed your project (outlined in Form A), use this form to submit your final report.
• Form E: Request for Withdrawal of Local League Ed Funds
  Use this form to request withdrawal of funds.
• Form F: Deposit Transmittal Request Form
  Use this form to deposit money (i.e., donations or grants) into your local League Ed Fund account.
• Form CE1: Civic Education Grant Form
• Form CE2: Civic Education Grant Final Report Form
